Community Registered Nurse


Description

Right at Home Newcastle Toronto provides in-home care to older people across Western Lake Macquarie, and we are looking for a Registered Nurse to join us on a casual basis.

This is nursing in the community, so you work one to one with people in their own homes rather than on a ward. Your days are mostly clinical: wound care, catheter care and insertion, medication administration, clinical reviews and the documentation that goes with them. You plan your own run of visits, with our Care Manager and office team behind you whenever you need them.

We would also like you to help with client assessments and care planning from time to time. That means sitting down with a client in their home, working out what they need and what matters to them, and writing the care plan our care workers then follow. It is not the bulk of the role, but it is one of the more interesting parts of it, and it suits a nurse who likes seeing the whole picture rather than one clinical task at a time.

What you would be doing:

* In-home nursing care, including wound care, catheter care and insertion, and medication administration
* Clinical reviews, and clear and timely documentation of the care you provide
* Working within each client's care plan, our policies, the Aged Care Quality Standards and the Code of Conduct for Aged Care
* Communicating with clients, families, GPs and allied health, along with our care workers and office team
* Supporting our care workers, who will often look to a nurse for guidance
* Occasional client assessments and care plan development
* Planning your day, including travel, reporting and administration

Pay is under the Nurses Award (MA000034) and set to your level of experience, with casual loading on top.

Qualifications

* Current AHPRA registration as a Registered Nurse, with no conditions that would stop you working on your own in someone's home
* A Bachelor of Nursing or an equivalent nursing qualification
* A current National Police Check.
* A current driver's licence and your own dependable car, comprehensively insured. 
* The right to work in Australia
* A real interest in aged care and in helping people stay in their own homes
* A good working knowledge of what applies to a nurse in home care, including the Aged Care Quality Standards, the Code of Conduct for Aged Care, and the rights of the people in our care
* Strong written and verbal communication.
* Confidence talking with clients and their families, and with GPs, allied health and our own care team
* A solid grasp of keeping clients, yourself and others safe, including emergency procedures and work health and safety
* Comfort working on your own in a client's home.
* Good documentation habits.
* Confidence using a computer for rostering, notes and reporting
* Keeping your CPD up to date as AHPRA requires

Preferred Skills

  • Experience in aged care or community home care. This is what we would most like to see, though we will consider strong applicants from other settings and we welcome new graduates
    * Confidence with wound care, catheter care and medication management in an unsupervised setting
    * Experience carrying out client assessments and writing person-centred care plans
    * Familiarity with the Aged Care Quality Standards, and with the Support at Home program
    * Additional qualifications or experience in dementia care, palliative care or geriatric nursing
    * Specialised skills such as tracheostomy care or dysphagia management
    * An eye for risk, and the judgement to know when something needs escalating
    * Patience and warmth with older people and their families, including in difficult conversations
    * Good time management, since you are managing your own run of visits and travel
  • * Seeing the client as a whole person and working through their care methodically.
