Palliative care offers vital support for families and carers, including bereavement assistance and support programs. Our goal is to help people live as fully and comfortably as possible, acknowledging that dying is a normal process and that good palliative care can ensure dignity and comfort.

At Right at Home, we believe end-of-life care is about more than just a care plan; it's about creating a supportive and peaceful environment with empathy, expert planning, and clear communication. Our teams coordinate fast-tracked access to services, collaborate with GPs and palliative care providers, and ensure care is delivered with dignity, respect, and compassion.
From 1 November 2025, the Australian Government's new Support at Home program will include an End-of-Life Pathway offering up to $25,000 in funding over 12 weeks and may be extended for 16 weeks for older Australians with a life expectancy of three months or less, providing urgent support via professional referral.
To access the End-of-Life Pathway, an individual must be 65 or older (or 50+ for Aboriginal and Torres Strait Islander peoples), have a confirmed life expectancy of three months or less, and have a Karnofsky Performance Status (AKPS) score of 40 or below, indicating significant functional decline. Eligibility requires a healthcare professional to complete and submit a form via My Aged Care for a fast-tracked assessment.
The End-of-Life Pathway provides a one-off budget of $25,000, available for up to 12 weeks, with no lifetime cap on access. While a person may only use the pathway once per period of end-of-life care, the funding is intended to support urgent and practical needs during this time. This budget is designed to help ease the pressure on families and ensure that care can be provided at home whenever possible.
The End-of-Life Pathway covers a range of essential services, including nursing care, personal care such as showering and toileting, domestic assistance, and meal preparation and delivery. It also provides funding for transport to and from appointments and assistive technology (if clinically prescribed).
