Right at Home Australia provides integrated chronic disease management through the Clinical Services category of the Support at Home program, offering fully government-funded, nursing-led care designed to stabilise complex conditions and prevent hospitalisation. By combining expert clinical monitoring and specialised allied health with daily Independence services, we create a seamless support system that manages the medical and practical challenges of living with long-term illness.

Fully Funded Clinical Support – Chronic disease care falls under the "Clinical" category of Support at Home. This includes regular nursing visits for medication management, wound care, and monitoring vital signs, as well as allied health services like dietetics and podiatry—all provided with zero client co-contribution.

Nursing-Led Oversight – Every client with a chronic condition has a personalised care plan managed by skilled nurses. They coordinate with specialists and GPs to ensure the condition is managed proactively, reducing the risk of hospital admission and/or readmission.

Integrated Multi-Disciplinary Care – Right at Home scales care as conditions progress. For example, a client with a chronic condition might receive Independence services (showering/transport) alongside Clinical services (nursing), ensuring a holistic approach to their daily and medical needs.
