Support at Home Care Partner / Coordinator

Location: Geelong, Victoria
Employment Type: Full-time
Salary: $45-$50 per hour and 12% Superannuation

Right at Home Geelong is growing, and we're looking for a passionate and motivated Support at Home Care Partner to join our team. This is an exciting opportunity to play a key role in delivering high-quality, person-centred care while helping shape the future of our expanding home care services.

As part of one of Australia's leading home care providers, you'll join a supportive, collaborative team committed to helping older Australians live independently and with dignity in their own homes.

About the Role

Reporting to the Clinical Care Manager, you'll coordinate and manage a diverse caseload of clients receiving Support at Home and Home Care Package services. You'll work closely with clients, families, care workers and health professionals to ensure every client receives responsive, high-quality care tailored to their individual goals.

This is a hands-on role that combines care coordination, client relationship management, clinical support and operational problem-solving. You'll enjoy variety, autonomy and the opportunity to contribute to the continued growth and success of our business.

Key Responsibilities

  • Build strong, trusting relationships with clients and their families.
  • Conduct client assessments, develop care plans and review services regularly.
  • Coordinate and monitor client services to ensure quality outcomes.
  • Match care staff to clients based on skills, availability and client preferences.
  • Support clients to achieve their individual goals and maximise their independence.
  • Monitor client wellbeing and respond promptly to changing care needs.
  • Liaise with GPs, allied health professionals and community service providers.
  • Identify opportunities for additional supports and make appropriate referrals.
  • Provide clinical guidance and support to care staff where required.
  • Respond promptly to enquiries from clients, families and team members.
  • Maintain accurate client documentation, incident reporting and care records.
  • Assist with rostering priorities and operational coordination in a dynamic environment.
  • Promote Right at Home services to referral partners and healthcare professionals.

About You

We're looking for someone who is compassionate, organised and proactive, with excellent communication skills and a genuine commitment to delivering outstanding client care.

You'll be comfortable managing competing priorities, building relationships with a wide range of stakeholders and working both independently and as part of a collaborative team.

Essential Qualifications

  • Certificate III or IV in Individual Support, Disability or a related qualification; or
  • Diploma of Nursing (Enrolled Nurse); or
  • Another relevant health or community services qualification.

Preferred Experience

  • Experience working in aged care or community care in a similar role of 3 years plus
  • Understanding of government-funded Home Care Packages and the evolving Support at Home program.
  • Knowledge of the SCHADS Award and Fair Work requirements.
  • Experience using VisualCare or similar client management and rostering software.
  • Strong assessment, care planning and case management skills.
  • Excellent organisational and time management abilities.
  • A positive attitude, willingness to learn and ability to adapt to change.
