We are looking for a compassionate and experienced Palliative Carer to join our team and provide high-quality, person-centred support to a client receiving palliative care in their own home.

About the role

You will provide personalised care and support in the client’s home, helping to maintain their comfort, dignity and quality of life. Duties may include:

• Providing compassionate personal care and assistance with daily activities
• Supporting comfort and wellbeing
• Monitoring and reporting changes in the client’s condition
• Supporting family members with a calm and reassuring approach
• Following individualised care plans and clinical instructions
• Maintaining accurate documentation and communicating with the wider care team
• Providing respectful, dignified end-of-life care

Qualifications

Essential

• Qualified Occupational therapist 
• Current AHPRA registered
• First Aid & CPR
• Australian National Police check
• Professional Indemnity Insurance
• Experienced in assessing and planning care needs within the community
• Valid Driver's Licence and reliable presentable car
• A smart mobile phone (iphone / android )
• COVID Vaccinations

Preferred Skills

• A passion for high quality client-focused care in a community setting
• Cert IV in Training and Assessment - highly desirable
• Exceptional communication skills
• Able to work autonomously or within a team environment
• Excellent time management and organization skills
•  Commitment to ongoing professional development
•  Residing in Western Australia is a requirement
