Right at Home is seeking a dedicated and experienced Clinical Care Partner (Registered Nurse) to support the delivery of high-quality, person-centred care to clients receiving services through Support at Home, NDIS, Private Services, and Brokerage Programs within the community setting.

The Clinical Care Partner plays a vital role in ensuring clinical excellence across all service streams through comprehensive clinical assessments, care planning, clinical governance, quality assurance activities, client reviews, staff mentoring, and the provision of nursing care. Working closely with clients, families, support staff, allied health professionals, and external stakeholders, the Clinical Care Partner ensures care is safe, compliant, responsive, and aligned with individual goals and needs.

This role combines clinical leadership with hands-on nursing expertise and contributes to the continuous improvement of service quality, client outcomes, and workforce capability.

The role includes an after-hours on-call roster, the on-call roster is shared across the team.

Essential

• Current registration as a Registered Nurse (RN) with the Australian Health Practitioner Regulation Agency (AHPRA).
• Minimum 3 years' post-registration nursing experience.
• Current National Police Check.
• Current First Aid and CPR certification.
• Current Australian Driver's Licence and reliable vehicle.
• Evidence of relevant vaccinations in accordance with organisational and legislative requirements.
• Full Australian working rights.

Desirable

• Experience within community nursing, aged care, disability services, or primary healthcare.
• Experience working with Support at Home, Home Care Packages, NDIS, private clients, and brokerage services.
• Qualifications or additional training in wound care, chronic disease management, dementia care, continence management, or palliative care.
• Certificate IV in Training and Assessment (TAE) or experience delivering staff education and mentoring.

• Strong clinical assessment and care planning skills.
• Sound understanding of clinical governance, quality assurance, and risk management principles.
• Knowledge of the Aged Care Quality Standards, NDIS Practice Standards, and relevant legislation.
• Demonstrated ability to manage complex clinical cases within a community setting.
• Excellent communication and relationship-building skills with clients, families, and stakeholders.
• Strong organisational skills with the ability to prioritise and manage competing demands.
• Experience conducting client reviews, service reviews, and clinical audits.
• Ability to mentor, coach, and support multidisciplinary care teams.
• High level of computer literacy and experience using client management and clinical documentation systems.
• Commitment to person-centred care, continuous improvement, and achieving positive client outcomes.
• Ability to work independently while contributing effectively as part of a collaborative team.
