We’re looking for a caring, reliable, energetic, and organised RN / Care Coordinator to become part of our growing team and help us deliver exceptional care across our local community.

 About the Role

As our RN / Care Coordinator, you’ll play a key role in making sure our clients receive the right care, at the right time, in the comfort of their own homes.

Your day-to-day will include:

•  Coordinating and overseeing in-home care services
•  Conducting care assessments, reviews, and service planning
•  Building strong relationships with clients, families, carers, and the wider care team
•  Monitoring care delivery to ensure quality, safety, and compliance
• Supporting carers and staff with guidance, training, and regular updates
• Travelling throughout the Wide Bay and surrounding areas — with a company car provided for work-related travel

What We’re Looking For

We’re looking for someone who is caring and capable, who genuinely enjoys connecting with people and takes pride in making a difference.

You’ll ideally bring:

•  Previous experience in Aged Care, Community Care, or Disability Support
•  Strong time management and organisational skills
•  Excellent communication and interpersonal skills
• Knowledge of care standards, assessments, and service coordination
•  A current driver's licence and access to a reliable vehicle
•  A genuine commitment to our purpose: “Improving the quality of life for those we serve.”

• AHPRA Registered and practicing 
• Drivers Licence
• Any other Aged Care / Health Care related certificates 
• Proven experience in aged care, disability, or mental health within a community setting.
• Strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
• Current First Aid, CPR certification, and National Police Check.
• Working With Children (WWC) check or willingness to obtain.
• A valid driver's license and access to a reliable car with comprehensive insurance.

• Knowledge of Home Care Packages (HCP) or Support at Home (SAH) (Desirable)
