Enrolled Nurse / Clinical Care Partner

Right at Home Adelaide Central is seeking an experienced and compassionate Enrolled Nurse / Clinical Care Partner to join our growing clinical team.

This is an exciting opportunity for an experienced Enrolled Nurse who is passionate about supporting older Australians to remain independent, safe, and well within their own homes.

Working closely with our Registered Nurses and multidisciplinary team, you will undertake client assessments, contribute to care planning, provide clinical support, coordinate services, and help deliver exceptional community-based care under the Support at Home Program, Home Care Packages and private services.

Key Responsibilities

• Conduct client assessments within the Enrolled Nurse scope of practice.
• Assist in developing and reviewing individualised care plans.
• Monitor client wellbeing and identify clinical risks.
• Support reablement and wellness-focused care.
• Provide clinical support and guidance to care staff.
• Participate in medication management processes.
• Deliver wound care, continence management and other nursing interventions within scope.
• Maintain accurate clinical documentation.
• Participate in multidisciplinary case conferences.
• Support incident management, quality improvement and compliance activities.
• Assist with onboarding new clients and care coordination.
• Build strong relationships with clients, families and health professionals.

• Qualifications

  Essential

  • Current AHPRA Registration as an Enrolled Nurse.
  • Minimum three years' nursing experience.
  • Community Nursing, Home Care, Support at Home or Aged Care experience.
  • Strong clinical assessment and documentation skills.
  • Excellent communication skills.
  • Current National Police Check.
  • Current First Aid and CPR Certificate.
  • Current unrestricted Driver's Licence.
  • Reliable motor vehicle.
  • Comprehensive vehicle insurance.
  • Full Australian Working Rights.

  • Experience with Support at Home Program.
  • Knowledge of Aged Care Quality Standards.

  • Wound care experience.
