Right at Home Gold Coast is expanding, and we are seeking experienced Case Managers, Care Managers and Care Coordinators to join our team as Care Partners across our Pimpama and Bundall offices.
If you have strong case management experience and enjoy building meaningful client relationships, coordinating services and supporting positive client outcomes, we would love to hear from you.
As a Care Partner, you will manage a portfolio of clients and coordinate their day-to-day care and support needs, ensuring services are safe, personalised and responsive to changing needs.
Key responsibilities include:
Manage and coordinate a portfolio of clients and their services.
Conduct client reviews, welfare checks and ongoing care monitoring.
Assess client needs and develop, review and update care plans.
Coordinate services with clients, families, Care Workers and external providers.
Monitor service quality, client outcomes and changing care needs.
Respond to client enquiries, feedback and complaints.
Maintain accurate client records and ensure compliance with relevant legislation and organisational requirements.
Support continuous improvement and the ongoing development of our Support at Home services.
You will ideally have:
Previous experience in Case Management, Care Management, Care Coordination or Community Aged Care.
Nursing qualifications are highly valued.
Experience managing a client portfolio and coordinating community-based services.
Knowledge of the Australian aged care sector and Support at Home.
Certificate III in Individual Support/Aged Care or a higher relevant qualification.
Strong communication, organisational and problem-solving skills.
Excellent documentation and computer skills.
Ability to work independently and manage competing priorities.
Current Australian Driver Licence.
Current or willingness to obtain an Australian Federal Police Check.
Full-time Monday–Friday position
8:30am–4:30pm hours
