Care Coordinator

Description

We are seeking an experienced and organised Care Coordinator to oversee the coordination and delivery of in-home care services for our clients across the Lower North Shore. This role is essential to ensuring continuity of care, client satisfaction, and compliance with regulatory and organisational standards.

The Care Coordinator works closely with clients, families, care workers, and internal teams to ensure services are delivered efficiently, safely, and in line with individual care plans. 

Client Care & Service Delivery

  • Coordinate and manage client care services to ensure high-quality, person-centered care delivery

  • Conduct initial client intakes, assessments, and reviews in collaboration with the care management team

  • Develop, implement, and maintain individualised care plans aligned with client needs, goals, and funding requirements

  • Monitor service delivery and proactively address changes in client needs or circumstances 

Compliance & Quality Assurance

  • Ensure all services comply with Aged Care Quality Standards, NDIS Practice Standards (where applicable), and organisational policies

  • Maintain accurate and up-to-date client and care worker records within scheduling and care management systems

  • Monitor service delivery to ensure compliance with care plans, risk assessments, and safety requirements

  • Participate in audits, quality improvement initiatives, and incident management processes

Communication & Stakeholder Engagement

  • Liaise with clients, families, health professionals, and external stakeholders as required

  • Escalate risks, incidents, and service concerns appropriately

  • Work collaboratively with Care Managers, Office Staff, and Management to ensure seamless service delivery

Administration & Reporting

  • Maintain accurate documentation, notes, and service records

  • Support reporting requirements related to service delivery, incidents, and compliance

  • Assist with funding utilisation monitoring and service adjustments

Qualifications

Qualifications (Deal Breaker)

  • Certificate IV or Diploma in Aged Care, Community Services, Disability, or related field

  • Relevant industry experience may be considered in place of formal qualifications — At least 1-2 years of experience

  • Valid Australian Drivers license

Skills & Experience

  • Previous experience in a Care Coordinator, Scheduling, or similar role within home care, aged care, disability, or community services

  • Strong understanding of person-centred care principles

  • Knowledge of Aged Care Quality Standards and/or NDIS requirements (desirable)

  • Excellent organisational, time management, and problem-solving skills

  • Strong communication and interpersonal skills

  • Ability to manage competing priorities in a fast-paced environment
