As a Care Partner, you’ll be the key contact for clients and their families, coordinating support services under the Support at Home framework. You’ll work across a variety of services, including allied health, nursing, home support, and equipment provision, empowering clients to live life on their terms.
This is a full-time, office-based role with a company vehicle provided for all client-related travel.
Build trusted, meaningful relationships with clients and their families.
Develop holistic care plans and coordinate multidisciplinary services.
Collaborate with clinical teams, allied health providers, and external stakeholders to deliver safe, high-quality care.
Support clients through care transitions such as hospital discharge or changes in health needs.
Identify opportunities to enhance client outcomes and contribute to service growth.
Champion safety, compliance, quality, and continuous improvement initiatives.
Experience in case management, care coordination, or community/aged care.
Strong customer focus and the ability to build relationships with empathy and professionalism.
Excellent written and verbal communication; confident with Microsoft Office and CRM systems.
Knowledge of aged care and NDIS funding streams.
Strong decision-making, problem-solving, and time management skills.
Ability to work collaboratively with families, clinicians, and external providers.
Relevant qualifications such as Certificate IV or Diploma in Community or Aged Care (desirable).
Current Australian Driver’s Licence (essential).
Work-life balance: Full-time, Monday to Friday – no weekends.
