With the new Support at Home program launching 1 November 2025, this is a truly exciting time to join the sector.
You’ll help clients navigate national reform, tailor services to their unique needs, and support the rollout of a program designed to empower older Australians like never before.

What You'll Be Doing 

You'll be a key part of our client care journey bringing heart, clarity, and coordination to every step. 

Your Role includes:

• Connecting with clients, families & health professionals
• Coordinating services and crafting individualised care plans
• Ensuring high-quality, person-centered care delivery  
• Supporting carers in the field and educating where needed 
• Client budgets and inquiries 

What We're Looking For

• Experience in care coordination, case management or similar 
• Background in aged care, disability support, or community services 
• Brilliant communication, time management & problem-solving skills
• A strong ethical approach and keen eye for detail 
• Confidence with tech & care management 
• A current driver's licence + reliable car (work vehicle discussed during interview)

• Diploma-qualified nurses (RN/EN) highly encouraged to apply!

• Cert IV or Diploma in Aged Care, Disability, Community Services (preferred)

• Knowledge of formally known as the Home Care Packages (HCP) now known as (SAH) Support at Home 

• Current drivers licence + fully comprehensive car insurance 

• Current police check within the last 12 months

• Current First aid & CPR
