About the Role:

As an EN / Care Coordinator, you will:

Coordinate and oversee in home care services
Conduct care assessments, reviews, and service planning
Collaborate with clients, families, and the care team to ensure individual needs are met
Monitor care delivery to ensure quality, safety, and compliance
Support carers and staff with guidance, training, and updates
Willing to travel wide bay and surrounding areas (Company car provided)

What We’re Looking For:

We’re seeking someone who is both caring and capable someone who thrives on connection and brings a high level of professionalism to everything they do.

You will ideally have:

Previous experience in Aged Care, Community Care, or Disability Support
Strong time management and organisational skills
Excellent communication and interpersonal abilities
Knowledge of care standards, assessments, and service coordination
A current driver's licence and access to a reliable vehicle
A genuine commitment to "Improving the quality of life for those we serve"

Qualifications

AHPRA Registered and practicing
Drivers Licence
Any other Aged Care / Health Care related certificates
Proven experience in aged care, disability, or mental health within a community setting.
Strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
Current First Aid, CPR certification, and National Police Check.
Working With Children (WWC) check or willingness to obtain.
A valid driver's license and access to a reliable car with comprehensive insurance.
