About the Role:

As an EN / Care Coordinator, you will:

  • Coordinate and oversee in home care services 
  • Conduct care assessments, reviews, and service planning 
  • Collaborate with clients, families, and the care team to ensure individual needs are met
  • Monitor care delivery to ensure quality, safety, and compliance 
  • Support carers and staff with guidance, training, and updates 
  • Willing to travel wide bay and surrounding areas (Company car provided)

What We’re Looking For:

We’re seeking someone who is both caring and capable someone who thrives on connection and brings a high level of professionalism to everything they do.

You will ideally have:

  • Previous experience in Aged Care, Community Care, or Disability Support 
  • Strong time management and organisational skills 
  • Excellent communication and interpersonal abilities
  • Knowledge of care standards, assessments, and service coordination
  • A current driver's licence and access to a reliable vehicle 
  •  A genuine commitment to "Improving the quality of life for those we serve"

Qualifications

  • AHPRA Registered and practicing 
  • Drivers Licence
  • Any other Aged Care / Health Care related certificates 
  • Proven experience in aged care, disability, or mental health within a community setting.
  • Strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
  • Current First Aid, CPR certification, and National Police Check.
  • Working With Children (WWC) check or willingness to obtain.
  • A valid driver's license and access to a reliable car with comprehensive insurance.

Preferred Skills

  • Knowledge of Home Care Packages (HCP) or NDIS (Desirable)
