Case Manager for Home Care Packages and NDIS

Position Overview:

We are seeking a dedicated and experienced Case Manager to join our team. The Case Manager will be responsible for coordinating and managing home care packages and National Disability Insurance Scheme (NDIS) services for our clients. This role requires strong organizational skills, excellent communication abilities, and a passion for making a difference in the lives of others.

Responsibilities:

  • Conduct comprehensive assessments of clients' needs and develop individualized care plans in collaboration with clients, their families, and other healthcare professionals.
  • Coordinate the delivery of home care services, including personal care, household assistance, nursing care, therapy services, and other support services as required.
  • Liaise with NDIS participants and their support networks to ensure services are delivered in accordance with their NDIS plans.
  • Monitor and review clients' progress, adjusting care plans as necessary to meet changing needs and preferences.
  • Provide guidance and support to clients and their families, empowering them to make informed decisions about their care and support needs.
  • Maintain accurate and up-to-date documentation, including client records, care plans, progress notes, and service agreements.
  • Collaborate with internal teams and external stakeholders to ensure the seamless delivery of services and the achievement of positive outcomes for clients.

Qualifications

  • Bachelor's degree in social work, nursing, disability studies, or a related field (preferred).
  • Previous experience in case management, preferably in the home care or disability sector.
  • Strong understanding of the NDIS and home care packages, including eligibility criteria, funding arrangements, and service delivery requirements.
  • Excellent communication and interpersonal skills, with the ability to build rapport and establish positive relationships with clients, families, and stakeholders.
  • Highly organized with the ability to manage multiple tasks and prioritize workload effectively.
  • Compassionate, patient, and empathetic approach to client care.
  • Proficient computer skills, including experience with client management software and Microsoft Office applications.
