Care Manager - Registered Nurse (Home Care)

Right at Home Sydney Inner West are seeking a qualified and experienced RN Care Manager (Full Time: Mon-Fri 9am-5pm) for our in-home care company. Our office provides Home Care, Nursing and Allied Health services to clients from Carlingford to Concord/Burwood, including Homebush, Auburn, Lidcombe and surrounding suburbs. Our team is committed to person‑centred care, operational excellence and maintaining a supportive workplace culture.

Right at Home is a leading provider of in‑home care services across Australia and internationally. We support older people, individuals with disabilities, and people with complex or post‑hospital care needs to live independently and safely in their own homes. We provide services under a range of funding sources including private pay clients and My Aged Care - Support at Home Program (including Restorative Care and End of Life Pathways).

This role is responsible for:

  • Conducting in-home clinical visits, assessments and care plans to best meet client needs.
  • Provision of in-home nursing services, including medication administration, catheter care and wound care.
  • Managing client compliance and a case load of clients as a Clinical Care Partner (including End of Life and RCP clients)
  • Handling information requests, feedback, incidents, general enquiries and complaints.
  • Training, and managing the performance of the care and nursing team.
  • Ensure all documentation is accurately and comprehensively completed in Right at Home databases and software programs.
  • Travel within service area to clients homes to complete client assessments and services.
  • Liaison with clients, families, and health professionals ensure continuity of care
  • Participates in continuous improvement, including monitoring of clinical indicators and follows risk management open disclosure frameworks
  • Answer new client enquiries over the phone in a knowledgeable manner with the aim of scheduling a care consultation
  • Participating in after-hours on-call phone duties every 2 to 3 weeks as part of the rotating roster system.

Qualifications

Essential Criteria:

  • Bachelor Degree in Nursing
  • Current registration with AHPRA
  • Professional Indemnity Insurance.
  • A minimum of 1-2 years of case management or care coordination experience is required.
  • Current Australian National Police check & Working with Children Check
  • Current First Aid & CPR certificate
  • Driver's license and your own reliable, registered vehicle (Essential)
  • Australian citizenship or permanent residency status is required for this position.
  • CATHETER CARE COMPETENCY & EXPERIENCE IS ESSENTIAL (SPC & IDC)

Ideally you will have:

· Excellent problem solving, decision making skills and highly developed interpersonal skills. You can effectively communicate at all levels enabling you to achieve best nursing practices in accordance with legislation, best practice and quality improvement.

· A passion for community care and the ability to work effectively with people from different cultural backgrounds;

· Ideally 2 – 3 years' experience working within the community care sector

· Detailed understanding of the Home Care industry and Aged Care Standards is highly regarded.

· Responsible delivery of medication

· Proficiency and confidence in using technology daily
