Right at Home Gosford is seeking a Care Partner to join our local team and play a crucial role in providing high-quality in-home care services to our valued clients. If you have a strong background in aged care and community services, excellent communication skills, and a commitment to enhancing the well-being of seniors and vulnerable individuals, this opportunity is for you. 

Our preferred candidate will demonstrate client-centred practice, be a natural relationship builder, and reliable with excellent organisational skills; making you a great asset to our team. 

Your new role will be responsible for: 

  • Meeting with clients and their families, reviewing their welfare and building strong connections 

  • Implementing and reviewing care plans to best meet client requirements 

  • Handling information requests, feedback, and general enquiries across a variety of stakeholders  

  • Ensuring all documentation is accurately and comprehensively completed in Right at Home databases and software programs 

Qualifications

To be successful in this role, you will need: 

  • Relevant qualification in Aged or Community Care and experience with care coordination

  • Highly developed verbal and written communication skills 

  • Strong organisational, problem solving and time-management skills 

  • Proficient computing skills

  • Ability to manage client funding budgets  

  • Current Australian National Police Check and First Aid & CPR certificate  

Preferred Skills

  • Sound understanding of the Aged Care Support at Home funding model

  • Demonstrated experience supporting care staff
