About the Role

As a Care Partner, you will oversee and coordinate the delivery of high-quality home care services to clients across the Sutherland Shire.

This is a full-time, onsite position based in our Miranda office, with community travel required.


In This Role, You Will:

  • Conduct comprehensive client assessments

  • Develop and review personalised care plans

  • Manage a caseload of Home Care Package and/or NDIS clients

  • Ensure compliance with funding, regulatory and organisational requirements

  • Coordinate care services in line with allocated budgets

  • Liaise with clients, families, support workers and allied health professionals

  • Manage documentation and reporting requirements

  • Respond to incoming enquiries in a professional and timely manner

  • Build referral pathways and maintain relationships with external providers


Must Haves

  • Relevant qualification (Cert IV or Diploma in Nursing highly regarded)

  • Experience in aged care, Home Care Package, NDIS or community care coordination

  • Strong understanding of My Aged Care and/or NDIS systems

  • Proficiency in Microsoft Office 365, Outlook and rostering systems

  • Valid Australian Driver’s Licence

  • Ability to travel across the Sutherland Shire

  • Authorised to work in Australia


Nice to Haves

  • Experience in mental health coordination

  • Previous case management experience

  • Established local referral networks

  • Knowledge of compliance and quality standards in home care
