Care Partner – Support at Home Team

(AIN Cert III / Enrolled Nurse)

Right at Home is seeking a dynamic and compassionate Care Partner to join our dedicated Support at Home team. This is an exciting opportunity for an experienced professional who is passionate about delivering high-quality care in the aged care and community sector.


About the Role

As a Care Partner, you will play a key role in coordinating and supporting care delivery within each client’s individual budget. You will ensure our clients receive person-centred, respectful, and effective services in their homes and communities.

You will be supported and mentored by Registered Nurses who are on call, providing clinical guidance, oversight, and professional support.

Key responsibilities include:

  • Supporting and coordinating care services in line with care plans and funding allocations
  • Providing leadership, guidance, and support to care staff
  • Monitoring service delivery to ensure quality, safety, and compliance standards are met
  • Liaising with clients, families, and multidisciplinary teams
  • Maintaining accurate documentation and reporting

About You

To be successful in this role, you will bring:

  • A minimum Certificate III in Assistant in Nursing (AIN) or be an Enrolled Nurse (desirable)
  • 2–3+ years’ experience in a team leader or supervisory role
  • Strong communication and interpersonal skills
  • Excellent time management and organisational abilities
  • Demonstrated experience in the aged care or community services sector

Mandatory Requirements

  • Current Driver’s Licence
  • Access to a reliable, fully insured vehicle (comprehensive insurance required)
  • Access to a mobile phone
  • National Criminal History Check (within the last 3 years)
  • Current First Aid Certificate (within 3 years)
  • Current CPR Certification (updated annually)
  • Australian Citizenship or valid working rights in Australia
