As a fast-growing organisation, we are seeking a passionate and values-driven Care Partner to join our Northern Rivers team.
The Care Partner is a key role within our organisation, working closely with the Care Manager to coordinate and oversee the delivery of high-quality, consumer-directed care services. You will support a diverse client base across Home Care Packages / Support at Home, NDIS, and Private Pay services.
This position combines client advocacy, care coordination, team support, compliance, and relationship management, ensuring services are delivered in line with legislative requirements, funding guidelines, and Right at Home values.
This is a full-time, Monday to Friday role, office-based, with participation in a rotating on-call roster.
As a Care Partner, you will:
Manage and coordinate a portfolio of clients
Act as the primary point of contact for clients and families, supporting:
Rostering and service coordination
Budget monitoring and service utilisation
Purchases, reviews, and ongoing care planning
Support and guide care staff through:
Day-to-day operational advice
Mentoring and training oversight within scope
Conduct client reviews, and non-clinical assessments using approved tools
Monitor client wellbeing and escalate clinical concerns to the clinical team as required
Advocate for clients, ensuring their preferences and goals remain central to decision-making
Liaise with allied health professionals, clinicians, families, and external stakeholders
Ensure compliance with Aged Care, Disability, Support at Home, and NDIS standards
Contribute to continuous improvement, quality indicators, and organisational outcomes
Provide nursing support where applicable (EN/RN applicants only)
Participate in a rotating on-call roster to support after-hours operational needs

You will hold one of the following:
Certificate III or IV in Aged Care, Disability, Individual Support, or a related field or
Enrolled Nurse (EN) with current AHPRA registration or
Registered Nurse (RN) with current AHPRA registration
You will also have:
A minimum of 1-2 years of experience in community-based aged care and/or disability services
Understanding of consumer-directed care, funding models, and quality standards
Knowledge of clinical governance and continuous improvement frameworks
Current First Aid and CPR certification
National Police Check
Current Australian Driver's Licence
Strong computer literacy and confidence using CRM systems
Excellent communication, organisation, and problem-solving skills
Ability to multitask, prioritise, and remain calm under pressure
A collaborative, proactive attitude and commitment to service excellence
Willingness to participate in a rotating on-call roster
Current Australian work rights are required for this position
Previous experience in care coordination or care partnering
Proficiency in Microsoft Word and Excel
Confident and professional telephone manner with a strong customer service focus
Ability to build and maintain referral and stakeholder relationships
Understanding of home care guidelines and current sector reforms
Confidence in navigating challenging or sensitive conversations
