Are you an experienced Occupational Therapist looking to make a genuine impact in community-based aged care?
At Right at Home Brisbane South, we pride ourselves on delivering premium, highly personalized in-home care. As the aged care landscape evolves under the 2024 Aged Care Act, we are seeking a dedicated Occupational Therapist to join our clinical team and help our clients thrive under the Support at Home program.
This is a community-based role where you will have the autonomy to manage your schedule across the Southern Brisbane, supported by a highly organized administrative team and forward-thinking leadership. Please note: We are exclusively focused on the aged care sector and are not an NDIS provider.
Working within the Support at Home framework, you will provide vital "clinical supports" to help older Australians maintain their independence and safety at home. Your day-to-day will involve:
Conducting comprehensive functional assessments in clients' homes.
Prescribing and applying for funding under the Assistive Technology and Home Modifications (AT-HM) scheme.
Developing and reviewing tailored care plans, including falls prevention strategies and mobility assessments.
To be successful in this role, you will need:
A minimum of 3 years of clinical experience as an Occupational Therapist (community or aged care experience is highly regarded).
Current and unrestricted AHPRA Registration.
A relevant tertiary qualification in Occupational Therapy.
A valid Australian Driver’s License and access to a reliable vehicle.
Strong digital literacy and the ability to adapt to modern health care models.
