Clinical Care Partner


Description

As a Clinical Care Partner, you will coordinate support activities under the Right at Home framework, empowering older Australians to remain independent and make informed decisions about their care. Services may include allied health, home support, nursing, and equipment provision.

Client Focus

 

·            Serves as a liaison between referral sources, families, carers and administrative staff members.

·            Conduct clinical onboarding assessment for clients using Right at Home-approved and standardised tools, within program-specific requirements.

·            Deliver and delegate clinical staff to ensure efficient onboarding of clients while attending to positive customer service.

·            Provide clinical advice to the care coordination and rostering team, ensuring a clinical response to escalating health concerns of clients.

·            Clinical support for client care plans to ensure compliance with Right at Home procedures.

·            Clinical service delivery of care, including but not limited to infection control, wound care, medication administration and monitoring of vitals.

·            Delivery of education to ensure competency trained staff and individualised education based on client needs.

·            Work with key allied health professionals/other clinicians to ensure individualised health care plans based on Chronic disease/Disability.

·            Monitor, evaluate and review care plans as required by client care needs.

·            Record and monitor significant health conditions in the client's home and ensure all documentation in RAH’s CRMs and other related internal systems.

·            Support Care Coordinators to liaise with and refer to allied health, dementia and continence services and other medical practitioners as required to support clinical service delivery.

·            Discuss end-of-life care options with clients.

·            Ensure antibiotic stewardship is upheld and clients have appropriate medication reviews.

·            Ensure all client contact is managed in an empathetic, friendly, interested and patient manner to quickly build rapport, trust and confidence in Right at Home to ensure a positive and professional client experience every time.

·            Use a variety of superior interpersonal communication skills to deliver genuinely respectful client engagement including active listening, gaining all customer details, information gathering, problem analysis and solving, persuasiveness, and negotiation.

·            Provides client and family education on the nature of services that are available and the effect and benefit of the care plan.

·            Prepares in-home care assessments and care plans to best meet client requirements as directed by the Care Manager/Clinical Care Manager.

·            Performs the duties of a registered nurse as needed.

·            Assist in the monitoring of client care.

·            Engages in studies and investigations related to improving personal and nursing care.

·            As directed by the Care Manager/Clinical Care Manager, approves the supply of medical devices and aids as required by clients from time to time. Negotiation with approved suppliers.

Administrative Functions

 

·            Interprets health, aged and disability care policies and regulations for carer and nursing staff and employees.

·            Participates in the assurance of compliance with local, state, and federal regulatory agencies related to aged and disability care services.

·            Implementing and maintaining all relevant operating CRM systems, policies & procedures & service conditions for caregivers, nurses & other care professionals.

·            Serves as a liaison with the accounts department, providing input on billing and care plans.

·            Compliance with WH&S regulations to ensure safe delivery of care

 

 

Business Functions

 

·            As delegated by the Care Manager/Clinical Care Manager, assists management with implementing an appropriate model of In-home Care.

·            Represent the Company at conferences, health fairs and other community service functions, and participate in civic and/or community committees as requested.

·            Actively networking with relevant industry bodies & professional contacts.

·            Assisting the business to achieve desired quality, safety & commercial outcomes.

·            Uses efficient and cost-effective approaches to integrate technology into the workplace. Supports and implements organisational technology changes.

·            Commits to a focus on continuous improvement of care processes and procedures.

·            Ensure opportunities and requests for new products and/or services are identified, recorded and reported to assist in future business development and planning.

·            Actively participate in the implementation of new services in line with the requirements of each program and the Right at Home business development plans.

·            Achieve organisational and operational quality indicators and results-oriented performance measures as determined.

·            Maintain excellent currency of skills and knowledge relevant to the Health, Disability and Aged Care sectors.
