About The Role:  

• Clinical in-home services include care plan assessment, medication, incontinence, wound, chronic disease, complex wound management, palliative care, health and diabetes management 

• Clinical delivery and supervision of non-clinical team members in a community setting 

• Intake processing, clinical care planning and reporting 

• Conducting client care assessments with a focus on Consumer Directed Care 

• Coordinate the provision of quality person-centred care according to the client's needs, service/care plans and available funding according to the client's budget 

• General administration for documentation relating to client care and in alignment with AHPRA regulations and legislation 

• On-call roster 

Essential 

• Qualified Registered Nurse 

• Current AHPRA registered 

• First Aid & CPR 

• Australian National Police check 

• Professional Indemnity Insurance 

• Experienced in assessing and planning care needs within the community 

• Valid Driver's Licence and reliable presentable car 

• A smart mobile phone (iphone / android ) 

• COVID Vaccinations 

• A passion for high quality client-focused care in a community setting 

• Cert IV in Training and Assessment - highly desirable 

• Exceptional communication skills 

• Able to work autonomously or within a team environment
