Are you an experienced Registered Nurse who is ready to take on more of a leadership role, while still being clinically hands on and achieving that elusive work life balance? If yes, this opportunity may suit you.

Right at Home Northern Rivers is an established community care service provider, including in-home nursing care, with a range of care services 24 hours per day, 7 days a week to our clients throughout. We are a passionate team who aspire to meet client / participant needs whilst maintaining person centred care.

Due to the escalating demands for our nursing services and our commitment to providing 24/7 care, we are expanding our team by creating a Full Time Monday to Friday Community Registered Nurse role that will be responsible for delivering community nursing care. This role includes HCP and NDIS, supporting complex client needs, nursing rosters, complex care and care plans that meet HCP and NDIS standards. The position is well supported by our team of nurses with varying clinical skills and experience.

About the role

Community Nursing is the special ability to care for someone in their own home and this important role is a combination of nursing and administration; you will be able to maintain your clinical skills by visiting clients while continuing to develop your leadership skills, providing guidance to our RNs, ENs and Support workers.

The role supports the team, including Care Partners and Schedulers with clinical enquiries as they arise.

Responsibilities:

  • Delivering community nursing services.
  • Support monthly clinical meetings.
  • Client / participant care planning – ensuring care delivery is specific to client / participant needs.
  • Training staff – clinical and non-clinical.
  • Assisting with afterhours clinical directions for our clients / participants as required.
  • Implementation of complex personalised care provision and service delivery, supporting onboarding, administration and assessments as needed.
  • Work collaboratively with the Management Team, other stakeholders in the community including Families, GP’S, Support Coordinators, Allied Health, Specialists, and Discharge Planners.
  • Uses a holistic approach to achieve optimal client / participant outcomes.
  • Ensure client / participant clinical competency aligns with standards of practice.
  • Understanding of Aged care standards and NDIS Practice standards, quality framework.
  • Requirement to travel within our Northern Rivers region.

Essential Criteria:

  • Current AHPRA registration.
  • Bachelor of Nursing.
  • Current CPR Certificate.
  • Australian Citizenship or Australian Working Visa with No Restrictions.
  • Australian Drivers Licence
  • Fully vaccinated against COVID-19.
  • Police Clearance.
  • NDIS Worker Screening Check or willing to obtain
  • Experience in Assessments and planning care needs.
  • Organisational, administrative and time management skills.
