About the role

Most of our clients rely on family members to help make care decisions and provide support at home. The Family Support Worker works with these families to understand the pressures they face, build their confidence and capacity to care, and connect them with the right services.

The role reports to the Care Manager (Registered Nurse) and the Director, and works collaboratively with our external Social Worker and allied health providers to support families in line with their care and support plans.

Key responsibilities

  • Meet with clients and families to understand their circumstances, relationships, strengths and challenges, and assess their family support needs.
  • Identify how the client’s care needs affect the family or household, including carer stress, isolation, family conflict, cultural or language barriers, and financial or housing pressures.
  • Develop family support plans with clear goals, working with the Care Manager to ensure they align with each client’s care plan and funding arrangements.
  • Review and update family support plans when there are changes in the client’s health, family circumstances or caring arrangements.
  • Provide practical guidance and emotional support to families dealing with ageing, dementia, disability, chronic illness or end-of-life care.
  • Help families understand and access services including Support at Home, My Aged Care, NDIS and Carer Gateway.
  • Develop and deliver information and education sessions for family carers on topics such as dementia support, carer wellbeing, respite planning and accessing community services.
  • Refer clients and families to appropriate external services, including Social Workers, counselling, financial counselling, dementia support and other community services, and follow up on outcomes.
  • Liaise with relevant health, community and welfare services to support families and promote awareness of available community resources.
  • Monitor each family’s progress against their support plan, maintain accurate case records and prepare reports for the Care Manager.

Qualifications and experience

  • AQF Diploma or higher qualification and relevant experience.
  • At least one year of experience supporting families, carers or vulnerable clients in a community, aged care, disability or family services setting.
  • Knowledge of Australian aged care and disability systems, including Support at Home, My Aged Care and the NDIS.
  • Experience assessing support needs, setting goals with families and maintaining accurate case records.
  • Strong communication, active listening and conflict-resolution skills, with the ability to work sensitively with people from diverse backgrounds.
  • Ability to work collaboratively with clients, families, health professionals and community service providers.
  • Current NDIS Worker Screening Check and National Police Check.
  • Current NSW driver licence and access to a reliable vehicle for client and family visits.
