About the Role

We are currently seeking a reliable, and capable Care Coordinator to join our team in Sydney Central and Eastern Suburbs.

The Care Coordinator is responsible for the effective coordination and management of client services under the supervision of Care Manager. This includes client assessments, care planning, service coordination, and case management.

The successful candidate will work closely with clients, families, support workers, health professionals, and internal management to ensure services are appropriately planned, coordinated, delivered, and reviewed. Qualifications

Key Responsibilities:

Client Assessment and Care Planning

  • Conduct assessments and reviews for clients
  • Assess clients' individual needs, preferences, goals, risks, and support requirements.
  • Develop, implement, and regularly review person-centred care plans.
  • Monitor changes in clients' health, wellbeing, circumstances, and support needs.
  • Coordinate appropriate services and make recommendations based on client requirements.

Case Management and Service Coordination

  • Provide ongoing case management and coordinate services to ensure quality and continuity of care.
  • Liaise with clients, families, support workers, health professionals, and other stakeholders.
  • Coordinate referrals and collaborate with external service providers where required.
  • Monitor service delivery and address issues or changes in client needs promptly.
  • Respond appropriately to incidents and emergency situations and escalate concerns in accordance with organisational procedures.

Selection Criteria

  • Diploma of Nursing, Community Services or equivalent preferred
  • Having experience of working as Personal Care Workers, Aged Care Workers, or Disability Support Workers in community or home care, or
  • Experience in case management, service coordination, care coordination, program development, or client assessment.

Mandatory Compliance Requirements

  • Current National Police Check.
  • Current First Aid and CPR certificates.
  • Current unrestricted Australian driver's licence, car insurance, and vehicle registration.
  • Current Working With Children Check and NDIS Worker Check.
  • Current COVID-19 vaccination.
