About the Role

As a Clinical Care Partner, you will provide clinical leadership, care coordination and oversight for clients receiving services through the Support at Home Program. You’ll manage your own caseload, conduct clinical assessments, develop care plans, support care staff, and ensure services align with each client’s classification, goals and available budget.

You’ll be a trusted clinical contact for clients, families, care workers and multidisciplinary partners ensuring care is appropriate, safe, coordinated and person‑centred.

Essential Requirements

• Current AHPRA Registered Nurse registration
• Experience in community nursing, aged care, rehab or palliative care
• Strong clinical assessment and care planning capability
• Understanding of Support at Home classifications, funding and service delivery
• Experience supervising or guiding care staff
• Current First Aid & CPR
• Driver’s licence and reliable vehicle
• Police and screening clearance
• Ability to work independently and travel to clients’ homes.
