Physiotherapist


Description

The Physiotherapist provides clinical assessment, intervention, and support to older adults living at home.  This role plays a key part in enabling older clients to maintain independence, safety, and wellbeing in their own homes. The Physiotherapist is also responsible for ensuring quality, evidence-based care that aligns with Aged Care Quality Standards. The Physiotherapist is a critical member of the multidisciplinary team helping support identified clinical need and MDT collaboration and supporting the care management team. The Physiotherapist will also assist with clinical care management, falls assessments, restorative care, reablement plans and care planning.

Client Focus

Clinical Care and Service Delivery

·            Conduct comprehensive physiotherapy and mobility assessments to identify clients’ abilities and care needs.

·            Develop and implement client-centred intervention plans to promote independence, participation, and quality of life. This may include but is not limited to exercise prescription, chronic pain management and gait/ balance, reablement and restorative care intervention plans.

·            Prescribe and recommend assistive equipment, mobility aids, in line with clinical and funding guidelines.

·            Provide education and training to clients, carers, and families on safe mobility, falls prevention, and use of adaptive equipment.

·            Maintain accurate clinical documentation, reports, and service records in accordance with AHPRA and organizational standards.

 

Clinical Leadership and Supervision

·            Provide supervision, mentoring, and professional support to care workers and allied health assistants.

·            Model best practice clinical standards and uphold ethical, person-centred care.

·            Facilitate peer review, case discussions, and clinical training sessions.

 Quality and Compliance

·            Ensure all practice aligns with the Aged Care Quality Standards, AHPRA guidelines, physiotherapy scope of practice and organisational policies.

·            Participate in quality improvement activities, audits, and incident reviews

Qualifications

·        Current AHPRA registration as a physiotherapist

·        Appropriate qualifications and a demonstrated commitment to the Aged Care Sector and the Home and community care environment.

Preferred Skills

Desirable

·        Knowledge of Support at Home, CHSP, and My Aged Care frameworks

·         Experience with mobility / falls assessments and Aged Care Funding documentation.

·        Understanding manual handling, dementia care, and falls prevention strategies.
