When you receive support at home, the quality of care doesn’t just depend on tasks and services — it depends on how well everything works together around you. At Right at Home, our Care Management model is designed to ensure your care is personalised, proactive, and coordinated to help you live safely and confidently at home.

Your Care Manager is your central point of contact — someone who understands you, stays in touch regularly, and ensures your care evolves as your needs do.

 

What Is Care Management?

Care Management is the ongoing coordination and monitoring of your services to ensure they reflect your goals, preferences and wellbeing needs over time.

Your Care Manager works with you, your family (if involved) and your care team to make sure your support truly meets your needs — not just now, but as they change.

They help with:

  • Care planning and service setup
  • Coordinating carers and allied health professionals
  • Monitoring care quality and satisfaction
  • Supporting reassessments and changes to care needs
  • Helping navigate Support at Home or Home Care Package requirements
  • Advocating for your preferences and independence goals

How Often Will My Care Be Reviewed?

You won’t need to wait for a problem before hearing from us. We check in proactively as part of your care management:

Type of Contact What Happens
First Visit Your Care Manager introduces your support workers and confirms service setup.
Monthly Check-Ins We contact you regularly to see how you’re going and to make adjustments if needed.
Quarterly In-Home Supervisory Visit A member of our care team meets with you to review your care quality, safety and wellbeing.
Annual Review Full reassessment and update of your care plan and future goals.
At Any Time If your needs or situation changes, you can request a review any time — we respond quickly.

Is Care Management Included in My Funding?

Most clients receiving government-funded services via Support at Home or a Home Care Package have Care Management included within their funding level.

Private clients can also access Care Management as part of their service package to coordinate support and assist with navigating the system.

If you’re unsure of your level of care management, your local Care Manager can explain this during a review.
