Role Overview: As an Enrolled Nurse, you will play a crucial role in delivering high-quality in-home care and assistance to our diverse client base. Your responsibilities include liaising with clients, family members, staff, and stakeholders to ensure services align with clients' needs, goals, and preferences.

This role involves assessment, planning, and review of care plans, with a focus on client-centric and consumer-directed services.

In addition to case management, you will manage inside and outside sales inquiries, as well as develop and maintain new and existing referral sources.

Qualifications

  • Demonstrated knowledge and experience in coordinating consumer-directed care services for Government-funded HCP clients.
  • Familiarity with the Aged Care Act.
  • Diploma of Nursing or equivalent
  • Advanced communication skills and the ability to model a positive culture through professional service.
  • Understanding of the changing and diverse needs of clients.
  • Reliable fully insured vehicle and valid driver's license.
  • Australian National Police check.
  • First Aid and CPR certification.
  • Experience in managing staff.
  • Professional Indemnity Insurance
  • Excellent networking and consultation skills to develop new referral sources and relationships with external organizations.
  • Experienced in assessing and planning care needs within the community.
