Care Partner – Home Care

Full-Time | No Weekends
Location: Bundall office (covering Gold Coast region)

We are looking for a compassionate and committed Care Partner to join our Bundall branch. In this role, you'll guide clients through their care journey, ensuring they receive safe, person-centred support that allows them to age well at home or in the environment of their choosing.

About the Role

As a Care Partner, you will coordinate support services under the Support at Home framework, empowering older Australians to remain independent and make informed decisions about their care. Services may include allied health, home support, nursing, and equipment provision.

This is a full-time role based in our Bundall office, with a company vehicle provided for all client-related travel. 

Key Responsibilities

  • Build trusted relationships with clients and support them to achieve their goals.
  • Develop holistic health plans and coordinate multidisciplinary services.
  • Collaborate with clinical teams, families, and external providers to deliver safe, person-centred care and optimal outcomes.
  • Support clients through care transitions, such as hospital discharge or changes in health needs, and manage offboarding when required.
  • Maintain accurate and timely documentation, participate in audits, and contribute to quality and continuous improvement initiatives.
  • Identify opportunities to enhance client outcomes through additional services and support service growth.
  • Champion safety, compliance, quality, and a culture of continuous improvement.

What You'll Bring

  • A minimum of 1-2 years of experience in community or aged care is required.
  • A strong customer focus and the ability to build trusted relationships with empathy.
  • Excellent written and verbal communication skills; confidence using Microsoft Office and CRM systems.
  • Knowledge of funding streams, quality standards, audits, and experience supporting care transitions (e.g., discharge planning).
  • Strong decision-making, problem-solving, time management, and adaptability in a fast-paced environment.
  • A collaborative mindset and the ability to work effectively with families, clinicians, and external providers.
  • Relevant qualifications (Certificate IV or Diploma in Community or Aged Care desirable).
