In alignment with the government’s Support at Home program, Right at Home provides care management as a mandatory core service focused on the ongoing coordination and personalised planning of your support. Our care management process ensures our services reflect individual preferences and adapt as your health needs evolve, with every detail outlined in a tailored Care Plan developed through regular consultation.
As your provider, we perform essential monitoring to identify risks and track health goals, delivering at least one direct care management activity – such as a supervisory visit or phone call – every month. To support these professional oversight services, we charge a transparent care management fee capped at 10% of your quarterly budget. We do not charge separate package management or basic daily fees, ensuring more of your funding goes directly toward the high-quality care you deserve.
