Home care coordination services are professional supports designed to organise and integrate health and community services, enabling older Australians to live independently and safely at home for longer. This role is vital for those with complex needs who must navigate multiple service providers and clinical requirements simultaneously. As of 2025, these coordination services are primarily delivered through two major government-funded frameworks – the Support at Home program for older Australians and the National Disability Insurance Scheme (NDIS) for those living with disability. Coordination services help ensure that care is seamless, goal-oriented, and adaptable to changing health conditions.

Under Support at Home, Right at Home provides Care Plan Management and Restorative Care Management.
