You're committed to the overall well being of your clients and their families. So are we.
You're committed to providing them with the best solution possible for everyone involved. So are we.
You're committed to finding them superior care. That's what we provide. And we provide it in numerous areas, depending on the level of care your client needs:
- Physical Assistance
- Skilled Nursing
- Special Care Situations
Our Promise to You
There are several important business practices that are fundamental to Right at Home:
We are responsive
You or your clients can contact us at any time. Care managers are available 24 hours a day, seven days a week, 365 days a year. Your clients are our clients. We both want the best for them at all times.
We are selective
We employ only the best caregivers and then we carefully put the right caregiver with the right client through a precise matching process. Once a caregiver and client are matched, we provide an extensive orientation on the client's needs to the caregiver.
We are flexible
If a family is unsatisfied with a particular caregiver, all it takes is a phone call. We can custom match another caregiver to a client at any time.
We are protected
We carry all of the necessary liability coverage so that all of our clients and their families are fully protected. We’re insured and we cover all employer-related expenses, including the security of work cover.
Our Promises to Your Clients
When it comes to the care of your clients, you have very high expectations. We share the same high expectations.
We do everything we can to achieve our mission for our clients and to fulfill our promises:
- We treat you like our neighbours, because you are.
- We listen, because your experience and input help us provide the care your clients need.
- We strive to provide the best experience possible, because you expect nothing less.
Supervised Care Transitions is an Option
There is an alternative that will enhance your current discharge practices. Many healthcare facilities recognise the value of supervised care transitions provided by trained, reputable home care providers to help prevent unnecessary readmissions.
Right at Home is at the forefront of these providers. Our RightTransitions program is structured to work with you, other healthcare providers, your patients and their families. We work to reduce readmission rates and lower costs associated with readmissions. Whether you currently have a transitional care program or not, our caregivers can improve your patients' recovery.
Our non-medical care model includes services necessary to help patients transition safely out of a clinic or hospital, including:
- Coordinating communication between providers
- Frequent follow-ups with families and discharge planners
- Medication reminders
- Transportation to physician appointments
- Preparing Meals
- Running errands
- Keeping homes clean and safe
