Clinical Care Coordinator


Description

Right At Home is looking for a Clinical Care Coordinator to join our Sydney Sutherland Shire Team! 

We are looking for an Enrolled or Registered Nurse to oversee and facilitate the provision of high-quality care and services to our clients and participants. Experience in Home Care and NDIS case management is preferential, but not essential. 

 

  • FTP with a competitive salary package (based on experience) 
  • On-going training with experience Care Managers 
  • Excellent Miranda Office Location – 2 Min walk from Miranda Station

Key Responsibilities: 

  • Perform client care assessments and oversee ongoing care planning. 
  • Manage client compliance and maintain a case load of clients. 
  • Coordinate the delivery of personalized care tailored to clients needs and care plans while adhering to allocated funding and budget requirements.  
  • Engage in general administrative tasks associated with the role. 
  • Build strong relationships with clients and their families, ensuring positive and professional communication. 
  • Monitor and assess the quality of care provided, identifying areas for improvement, and implementing changes as needed. 
  • Assist with inside sales enquiries and contribute to business development opportunities. 

 

To be successful in this role you should have: 

  • Current National Criminal History Record (Police Check) 
  • Current First Aid and CPR Certification 
  • APHRA Registration 
  • Professional indemnity Insurance 
  • Strong written and interpersonal skills   
  • Driver’s license and comprehensively insured private vehicle.
