Care Partner – Aged Care
$80,000 + 12% Super | Company Vehicle | Full-Time | No Weekends
Location: Ballina office (covering Northern Rivers region)
Right at Home Northern Rivers is seeking a compassionate, proactive, and client-focused Care Partner to join our team and play a key role in delivering high-quality, person-centred care from Ballina to Tweed Heads and surrounding areas.
With the upcoming reforms under the Support at Home Program, this role presents an exciting opportunity to transition into a more traditional case management position within aged care.
About the Role
As a Care Partner, you will coordinate support services under the Support at Home framework, empowering older Australians to remain independent and make informed decisions about their care. Services may include allied health, home support, nursing, and equipment provision.
This is a full-time role based in our Ballina office, with a company vehicle provided for all client-related travel.
Key Responsibilities
Deliver person-centred case management tailored to individual goals and needs
Develop and manage care budgets in collaboration with clients
Maintain a caseload with regular assessments and care plan reviews
Build strong relationships with clients, families, and care teams
Coordinate services including allied health, nursing, and home support
About You
You are a confident coordinator with strong communication and advocacy skills. You're passionate about improving the lives of older Australians and thrive in a collaborative environment.
Previous experience in aged care case management (HCP/CHSP)
Background in nursing or community care coordination
Excellent organisational and interpersonal skills
Ability to work autonomously and within a team
Valid Australian driver’s licence
Eligibility for a National Police Check and NDIS Worker Screening
Nursing qualifications (RN or EN) – highly desirable

Salary: $80,000 - $85,000 (based on qualifications/experience) + 12% super
Schedule: Full-time hours, Monday to Friday – no weekend work
