Key responsibilities

• Knowledge of Aged Care and Community Care Sector, Community Care Common Standards
• Proven ability to assess, plan, implement and evaluate care and services for clients in the In-Home Community Care environment
• Ability to undertake holistic assessments and create risk informed person-centred care plans
• Experienced in wound care, medication management, continence care
• Proficient in Microsoft Suite

About you

• Current Registration with AHPRA
• Minimum 2 years experience in Aged Care/Community Nursing including knowledge and understanding of Home Care Packages
• Experience in Wound and Catheter care
• Problem solving, negotiation and conflict resolution skills
• Compassion and enthusiasm, with a friendly approach
• Excellent written and verbal Communication skills 
• A current Australian driver’s licence
• Reliable vehicle with comprehensive insurance
• Current National Police check
• Current first aid and CPR certificate
