About the Role

We are currently seeking a dedicated and experienced Care Manager to lead our passionate team in the Northern Rivers region. Reporting to the General Manager and working closely with a collaborative leadership team, you will be responsible for overseeing clinical care services, supporting our clients, and mentoring the team of Care Partners.

Key Responsibilities

  • Lead and support the Northern Rivers care team to deliver person-centred care in line with Aged Care and NDIS Quality Standards.
  • Manage clinical governance, including assessments, care planning, incident management, and internal audits.
  • Mentor and guide clinical and care coordination staff, fostering a culture of continuous learning and excellence.
  • Work collaboratively with Directors, the General Manager, and Client Services Managers to meet strategic and operational goals.
  • Ensure compliance and uphold the highest standards of clinical care delivery.
  • Champion client independence and wellbeing by supporting them to live life on their terms.

About You

You’re an experienced, compassionate community clinical leader who:

  • Holds current AHPRA registration as a Registered Nurse (without restrictions).
  • Has strong experience in community care, including complex clinical needs, medication management, catheterisation, and diabetes care.
  • Understands compliance requirements within the Aged Care and NDIS frameworks.
  • Has a track record of leading high-performing teams and achieving KPIs related to care quality and outcomes.
  • Demonstrates excellent communication and relationship-building skills.
  • Is proactive, solution-oriented, and confident in decision-making.
  • Has previous experience in home care, disability support, or community health (highly regarded).
  • Is tech-savvy with strong documentation and digital literacy skills.

Essential Requirements

  • AHPRA Registration (RN – unrestricted)
  • Current CPR certification
  • Valid (unrestricted) Driver’s Licence
  • National Police Check (Aged Care/Vulnerable Persons)
  • NDIS Worker Screening Check (Yellow Card)
  • Completion of the NDIS Worker Orientation Module
  • Full working rights in Australia
