Right At Home is looking for a Clinical Care Coordinator to join our Sydney Sutherland Shire Team!
We are looking for an Enrolled or Registered Nurse to oversee and facilitate the provision of high-quality care and services to our clients and participants. Experience in Home Care and NDIS case management is preferential, but not essential.

Key Responsibilities:
- Perform client care assessments and oversee ongoing care planning.
- Manage client compliance and maintain a case load of clients.
- Coordinate the delivery of personalized care tailored to clients needs and care plans while adhering to allocated funding and budget requirements.
- Engage in general administrative tasks associated with the role.
- Build strong relationships with clients and their families, ensuring positive and professional communication.
- Monitor and assess the quality of care provided, identifying areas for improvement, and implementing changes as needed.
- Assist with inside sales enquiries and contribute to business development opportunities.
To be successful in this role you should have:
- Current National Criminal History Record (Police Check)
- Current First Aid and CPR Certification
- APHRA Registration
- Professional indemnity Insurance
- Strong written and interpersonal skills
- Driver’s license and comprehensively insured private vehicle.
