Care Coordinator

Right at Home is one of the largest home care providers in the world, with approximately 600 offices in eight countries around the world.

Our Cairns office continues to grow quickly and we are looking for another experienced and dedicated Care Coordinator to support our growth. The successful candidate will preferably have a strong background in Community Aged/Disability Care. The role itself is responsible for service development, coordination and delivery, including ongoing client assessments and development of care plans that deliver flexible and personalised in-home support. Direct nursing support to clients is also incorporated within the role, as such a current AHPRA-Nurse Registration is required (RN). The successful applicant will thrive on working in a close-knit team and have a passion for delivering the highest quality in client care.

This role does require some flexibility as some weekend supports are likely to be required, with this time to be offset as reasonable through the week.

This key role will focus on case management, nursing support, customer service, personal care work where required and general administration.

Major duties include:

  • Care Assessments;
  • Government-Compliant Care Management Plans;
  • Holistic outcomes driven Care Planning;
  • Operation of the Care Planning and other management systems;
  • Nursing and personal care supports; and
  • Quality assurance.

Qualifications

  • Minimum of Diploma in Nursing with a current AHPRA registration;
  • Current First Aid & CPR; and
  • Driver's License.

Preferred Skills

  • Good understanding of the Government funding environment for aged care (primarily CDC) & NDIS (preferred but not necessary);
  • Good understanding of the hospital discharge environment;
  • Subscribe to a culture of teamwork, caring and quality service;
  • Proven leadership abilities, professionalism, a good reputation & respect for our staff, clients and their families;
  • It is also essential to have good judgment, quick decision-making, excellent communication, staff management and administrative skills; and
  • Current National Police Clearance.
