NDIS Care Coordinator

We are seeking an experienced, enthusiastic and highly motivated NDIS Care Coordinator to coordinate, manage and administer the delivery of care services to our participants.
This role is responsible for:
- Conducting client care assessments and care planning
- Supervision and management of Care Staff
- Intake processing, clinical care planning and reporting
- Managing relationships with participants and families
- Coordinate the provision of quality person centred care according to the client's needs, service/care plans and available funding in accordance with budget
- General administration with respect to tasks performed
- Building relationships with local stakeholders
Preferred Skills:
- Previous experience with regard to managing NDIS participants
- Experience and qualification in Mental Health will be highly regarded
- Cert III or Cert IV (Aged/Disability Care)
- First Aid, CPR and Police Check are essential
- Experienced in assessing and planning care needs within the community
- Valid Driver's Licence
- Registered, comprehensivly insured vehicle.
