Care Coordinator - Rockhampton


Description

Right at Home Cental Queensland provides high quality in-home care to the elderly and those living with a disabillity who require assistance in their home. We require a passionate and dedicated Care Coordinator for our Rockhampton office to assist in the performance of the coordination of in-home care. Responsibilities will include:

  • Conducting Client Care Assessments and Care Planning
  • Supervision and management of Care Staff
  • Discussing and promoting Right at Homes services with medical professions and referral sources
  • Reporting to head office

Essential Criteria

  • Empathetic, passionate and caring
  • Experienced in assessing and planning care needs
  • Significant experience and understanding in the Australian Aged Care or Disability sector
  • A client focus which is based on their goals, preferences and needs
  • Demonstrated experience managing staff

Qualifications

We are seeking a qualified and experienced Care Coordinator with a minimum of Certificate III in Individual Support or Certificate III in Aged Care. 

Qualifications in HR, Psychology or Business Management would also be highly regarded.

Preferred Skills

  • Sound understanding of the principles of My Aged Care, Aged Care Reforms, Consumer Directed Care
  • Comprehensive Assessment and Case Management experience
  • Experience in provision of care and services to people from culturally and linguistically diverse backgrounds
  • Commitment to work within RAH Mission, Vision and Values
  • Capacity to take initiative and to be innovative
  • Highly developed organisational, administrative, business, people, and financial management skills with proven ability to determine priorities, coordinate activities and meet deadlines
  • Customer service experience (in the Aged Sector essential)
