Right at Home Gold Coast Northern Rivers is seeking skilled nurses to join our team in a community-based role, providing essential care to individuals at risk of hospitalisation.
This position offers flexibility, allowing you to set your own availability while making a meaningful impact on client health outcomes.
Key Responsibilities & Requirements:
✅ AHPRA Registration – Must be registered with the Australian Health Practitioner Regulation Agency (AHPRA).
✅ Relevant Qualifications – Nursing and/or Midwifery qualification required.
✅ Vaccination Compliance – Ability to provide evidence of vaccinations to meet QLD Health requirements.
✅ National Police Certificate – Current National Police Check required (within 12 months.)
✅ First Aid and CPR Certificate – Valid and up-to-date certification required.
✅ Australian Driver’s Licence & Reliable Transport – Ability to travel between client locations.
Essential Skills & Expertise:
Palliative Care – Provide advanced nursing care in complex situations, supporting interdisciplinary teams while managing medications and treatments.
Wound Care, Stoma, Tracheostomy & Enteral Feeding – Skilled in wound assessment, tracheostomy care, enteral feeding management, and patient monitoring.
Chronic Disease Management – Assess patients with chronic conditions, offering guidance on treatment options, symptom management, and lifestyle modifications.
Cognitive Impairment Support – Adapt care plans to address cognitive, behavioural, and functional changes while ensuring patient dignity and ethical considerations.
Clinical Documentation – Monitor, evaluate, and report patient data to assist with escalation, risk management, and continuous quality improvement.
Complex Personal Care – Deliver patient-centred care to support health, well-being, and quality of life.
