Care Coordinator - Community Aged Care
We are seeking an experienced Case Manager who is motivated, reliable, and passionate about aged care.
Our Organisation provides services under a range of funding sources including private pay clients, My Aged Care and NDIS.
About the role:
The primary responsibility of the role is to provide quality support to our Clients in Charters Towers.
The successful applicant will be working with a team of Case Managers and liaising with Clients, Family Members, Staff and a wide range of Stakeholders to ensure services are provided in accordance with the Client's needs, goals and preferences.
Assessment and care planning of client's individual needs, to provide Client focused and Consumer directed care with quality outcomes.
In addition to case management, this role will assist with the management of inside and outside sales enquiries by developing and maintaining new and existing referral sources.
