Care Manager


Description

Are you a dynamic leader seeking a new role and challenge working for a family friendly locally owned organisation, if so we want to talk to you!

This role is an executive position in the company and you will sit on the Executive Management Committee in assisting with best practice and clinical governance of client care. Care Management is a ‘mission critical’ responsibility for the delivery of care to the company’s clients. This includes the recruitment, training and quality control of carers and the related systems and processes and leading the care team. The role is multifaceted and client focused, managing a range of clients Aged/Disability and of all age groups. The position holder will expect to 

  • Inspire, motivate and provide leadership to their team
  • Assist in recruitment and retention of the care team
  • Commit and contribute to organisational as well as team objectives
  • Manage and drive Clinical Governance, best practice with a focus on continuous improvement 
  • Take responsibility for their own and other’s performance, development and safety
  • Apply superior communication, negotiation and problem-solving skills
  • Think and act strategically
  • Manage corporate risk and budgets effectively
  • Report to Managing Directors
  • Increase in growth of the business
  • Enquiries, in-home care assessments, supervisory vests and auditing of clinical team.
  • Strong background in Aged/Disability Care in the Community
  • Outstanding time management 
  • Analytic and report writing skills
  • Computer literacy - essential

In addition to Care Management this role will help develop relationships with hospitals, clinics and care facilities with a view to delivering care services to meet the market along with education on the nature of services available to clients.

Qualifications

  • Current AHPRA Registration
  • Experience in clinical management role leading care teams
  • Relevant tertiary qualification (Aged Care, Disability and/or Management)
  • Certificate in a specific nursing related discipline (Dementia care, geriatric nursing, palliative care, continence care) - Highly desirable
  • Cert IV Training and Assessment - highly desirable for in-house training
