Care Coordinator - Rockhampton


Description

Right at Home provides high quality in-home care to the elderly and those living with a disability who require assistance in their home. We require a passionate and dedicated Care Coordinators to join both our Rockhampton and Hervey Bay offices to assist in the performance of the coordination of in-home care. This will be a part time position with expected growth to full time.

The Care Co-ordinator will coordinate all allocated client related activities within the Right At Home service ensuring that a high quality of consumer directed care is delivered to all clients within the statutory requirements.

Care Co-ordinator is a ‘mission critical’ responsibility for the delivery of care to the company’s clients. This includes the, training and quality control of carers and the related systems and processes

This role is responsible for:

  • Delivery and supervision of non-clinical team members in a community setting
  • Intake processing, care planning and reporting
  • Conducting client care assessments
  • Supervision of care staff
  • Schedule appointments and in-home services to appropriate staff based on discipline, availability and geographical requirements
  • Coordinate the provision of quality person centered care according to the client's needs, service/care plans and available funding in accordance with budget
  • General administration with respect to tasks performed

Qualifications

 

  • Aged and disability qualification desired
  • First Aid & CPR are essential
  • Australian National Police check
  • Experienced in assessing and planning care needs within the community
  • Valid Driver's Licence and reliable presentable car
  • Essential Criteria
    • Must have a Sound understanding of the principles of My Aged Care, Aged Care Reforms, Consumer Directed Care
    • Must have Significant experience and understanding in the Australian Aged Care or Disability sector
    • Must be Empathetic, passionate and caring
    • A client focus which is based on their goals, preferences and needs
    • Must Demonstrate experience managing staff
    • Must have Experience in provision of care and services to people from culturally and linguistically diverse backgrounds
    • Must have a Commitment to work within RAH Mission, Vision and Values
    • Must have the Capacity to take initiative and to be innovative
    • Must have a Highly developed organisational, administrative, business, people, and financial management skills with proven ability to determine priorities, coordinate activities and meet deadlines
    • Must have Customer service experience (in the Aged Sector essential)

  • We are seeking a qualified and experienced Care Coordinator with a minimum of Certificate III in Individual Support or Certificate III in Aged Care. 
  • Qualifications in Nursing, HR, Psychology or Business Management would also be highly regarded.
