Care Coordinator (RN) - Waterloo


We are currently recruiting for a Care Coordinator - RN. Our services include aged and disability care, transitional care, post-operative care and palliative care. Right at Home is a leading home care provider globally.

This role is a full-time / part-time management position reporting to the Managing Director. The job would involve coordinating and allocating your care team and ensuring that client needs are met. You will also be responsible for the assessment of HCP and private clients.


  • Registered Nurse - AHPRA registered
  • Current NSW Driver's License (must have)
  • First Aid and CPR Certificate
  • Police Check

Essential Criteria

  • Sound understanding of the principles of My Aged Care, Aged Care Reforms and Consumer Directed Care
  • Comprehensive Assessment and Case Management experience
  • Experience in provision of care and services to people from culturally & linguistically diverse backgrounds Sydney Eastern Suburbs, City and Inner City area
  • Commitment to work within Right At Home Mission, Vision and Values
  • Capacity to take initiative and to be innovative
  • Highly developed organisational, administrative and financial management skills with proven ability to determine priorities, coordinate activities and meet deadlines
  • Excellent communication and interpersonal skills, both written and oral and strong computer literacy with Microsoft and database applications
