About the Role:

• Position: Enrolled Nurse Care Coordinator
• Location: Bundall, Gold Coast, QLD (Onsite)
• Hours: Full-time, Monday to Friday, 8:30 AM – 4:30 PM
• Reporting to: Care Manager
• Travel: Covering Gold Coast North & South regions

Key Responsibilities:

• Conduct client assessments and develop personalised care plans.
• Coordinate high-quality community care services and monitor client progress.
• Provide clinical guidance and support to care staff.
• Maintain compliance, accurate documentation, and care coordination.
• Work collaboratively with clients, families, and healthcare professionals.

What We’re Looking For:

• AHPRA-registered Enrolled Nurse with a passion for community care.
• Experience in aged care or community nursing (preferred).
• Strong knowledge of My Aged Care and Home Care Packages.
• Excellent communication, problem-solving, and organisational skills.
• Ability to work both independently and as part of a team.
