We are looking for a compassionate and proactive Care Partner to join our Right at Home Perth West Coast team, based in our Subiaco office.
In this rewarding role, you’ll support older Australians and people living with disability to live safely, independently, and meaningfully at home. You’ll work closely with clients, their families, and healthcare professionals to design and coordinate care plans that reflect each person’s unique goals, values, and preferences.
You’ll be responsible for managing your own caseload, ensuring that services are well-coordinated, person-centred, and responsive. This role suits someone who enjoys relationship-based care, problem-solving, and making a difference every day.

Certificate III/IV in Aged Care, Community Services, or relevant qualification in Nursing, Allied Health, or Social Work.
Experience in care coordination, case management, or service delivery within aged care, disability, or community care.
Current Australian driver’s licence and reliable vehicle.
Current First Aid and CPR certification.
National Police Check and Working With Children Check (or willingness to obtain).
