Company Background: Right at Home is a leading home care provider with over 600 offices around the world.  We provide a wide range of care services all designed to help our clients stay in their homes. Our home care services include companion, personal, nursing, respite, transportation, aged, disability, transitional, 24 hour, and palliative care.

The Ideal Candidate: The ideal candidate for our team will have experience in community aged and/or disability care. They will have existing relationships with care facilities, hospitals, and clinics to support business development opportunities. This person will have the ability to connect with others naturally and effortlessly and the confidence to market Right at Home services to potential clients and referral sources. The care coordinator will have experience managing the complex scheduling requirements for community care workers.  They will enjoy working in a close-knit family environment and will always be willing to go the extra mile to ensure the satisfaction of our clients, their families, and our team members. EN, RN, or OT desirable as this role will ideally have the skill to perform nursing and occupational therapy when needed.

Purpose of Position: This role is customer focused and involves consulting with families, preparing in home care assessments, and drafting care plans to best meet client requirements. This position is responsible for recruiting, training, and managing the performance of our carers and nurses. In addition to care co-ordination duties, this position will help develop relationships with hospitals, clinics and care facilities with a view of delivering care services to meet the market.

Duties include:

  • Managing the complex scheduling requirements for community care workers
  • Dealing with a range of seniors and frail elderly customers and their families on a daily basis. This role will handle anything from information requests, feedback, general enquiries to complaints
  • Ensuring all client contact is managed in an empathetic, friendly, interested and patient manner to quickly build rapport, trust and confidence in Right at Home.
  • Providing client and family education on the nature of services that are available and the effect and benefit of the care plan.
  • Preparing in-home care assessments and drafting care plans to best meet client requirements
  • Managing complex government funded cases as well as private pay clients
  • Recruiting, training, and managing the performance of the care team
  • Participating in community activities or committees as requested
  • Ensuring highest quality service is consistently delivered
  • Assisting management with business development opportunities across a range of referral sources
  • Operate and maintain care planning and scheduling technology
  • Ensure all documentation is accurately and comprehensively completed in Right at Home databases and software programs in accordance with organisation policies and procedures and legislative frameworks.
