Our Perth Midland office continues to grow, and we are on the lookout for a dedicated Care Coordinator who is passionate about facilitating exceptional care to our clients.  

Reporting to the General Manager, this key role is client focused and involves consultation and collaboration with clients, participants, families, other medical professionals and other community-based supports to develop and deliver high quality in home and community care.  

This role is responsible for regularly attending clients’ homes, building care and support plans to meet the clients’ needs and preferences and working closely with the Care Team to navigate exceptional client outcomes, conduct appropriate assessments, and identify client individualised care requirements. 

Skills & Experiences 

An ideal candidate will be able to demonstrate the following: 

  • Experience in a similar role for more than 12 months  
  • Excellent interpersonal skills  
  • Exceptional time management skills  
  • Ability to work in a team collaboratively and work autonomously without direct supervision  
  • Clear understanding of the aged care and disability industries  
  • Demonstrated administrative experience capable of managing challenging workloads 
  • Be approachable, friendly and empathetic 
  • Driven and motivated  
  • Client and solutions focused, an out of the box thinker 
  • Computer literate and able to pick up new systems quickly 

 

A successful candidate must be able to supply:  

  • A Cert IV in health or Registration as RN/EN/OT/PT  
  • Clean Australian federal police clearance  
  • Reliable vehicle 
  • Valid WA drivers’ licence  
  • Comprehensive car insurance 
  • Current first aid and CPR certificates
